WHO Milestone Checklist by Month: What to Actually Watch For

The WHO's milestone checklist tells you what most babies can do by a certain age. Here's how to actually use it — what it means, what it doesn't mean, and when to call your pediatrician.

WHO Milestone Checklist by Month: What to Actually Watch For

Milestones Are Not Deadlines

The WHO developmental milestone data is based on large multicountry studies of healthy children developing in optimal conditions. The milestones represent what most children can do by a certain age — not what all children must do on that date. Development is a range, not a schedule.

Milestones by Domain

Gross Motor

  • 2 months: Briefly lifts head during tummy time
  • 4 months: Pushes up on elbows; may begin rolling
  • 6 months: Rolls both directions; sits with support
  • 9 months: Sits without support; pulls to stand
  • 12 months: Cruises furniture; may take first steps (range: 9–15 months)

Fine Motor

  • 2 months: Briefly holds objects placed in hands
  • 4 months: Reaches toward objects; brings hands to midline
  • 6 months: Transfers objects hand to hand
  • 9 months: Emerging pincer grasp
  • 12 months: Refined pincer grasp; stacks two objects

Communication

  • 2 months: Coos; reacts to sounds
  • 4 months: Laughs; vocalizes back when spoken to
  • 6 months: Babbles; responds to name
  • 9 months: Points or gestures; imitates sounds
  • 12 months: Says 1–3 words (wide range — some typical children don't have words until 15–18 months)

Social and Emotional

  • 2 months: Social smile
  • 4 months: Recognizes and prefers familiar faces; laughs
  • 6 months: Expresses pleasure and displeasure clearly
  • 9 months: Separation anxiety; plays peek-a-boo
  • 12 months: Shows affection to caregivers

Red Flags Worth a Pediatric Conversation

Regardless of age: loss of a skill the baby previously had; no social smiling by 3 months; no babbling by 9 months; no response to name by 12 months; any concern about hearing. Tracking app milestone logging — noting when you first observe a new skill — creates a developmental record that is genuinely useful clinically.
